What is a Care Plan?
A Care Plan is simply a document that details, what you need assistance with, how you would like that assistance given and when it is given. It should document your normal routines i.e. what time you like to get up and go to bed, what sort of things you like to eat, how many sugars you have in tea/coffee, what your general likes and dislikes are. It will also include clinical information such as a list of allergies and the medication you are on, as well as your medical history that we should know about, who to contact in an emergency etc. This all helps to enable your Personal Home Carer to deliver the best service possible to you.
